I was recently mentoring someone who’s in the middle of an organizational design process. It can feel overwhelming, balancing strategy, execution, and people. I shared a simple framework I use, the Tech Leadership Org Design Guide (link below). A few takeaways: - Start with clear objectives, what should change in the next 12–18 months, and how you will measure success. - Use guiding principles; when trade-offs arise, these principles keep you aligned with your strategy and people. - Balance quick wins with long-term structure; sometimes you need both. Reflections from LUGPA (Part 3): Referral Headaches are Real! One of the most telling quotes at LUGPA: “Referral management is the next frontier.” I’ve heard over and over how practices are spending precious time sorting through faces, trying to catch urgent diagnoses, and fighting backlog. Referral workflows have been overlooked for too long. These workflows are mundane, but mission-critical. Ripe for transformation, referral intake is a perfect ground for AI to make a huge impact, accelerate patient access, and give time back to healthcare teams. CARE is a mindset that promotes healthy, human centered, empowering, and empathetic communication across all interactions, both positive and challenging. In a work setting it can be thought of this way: C – Connect Before Anything Build relationships, not just fix problems. Boss: “I’ve been meaning to say, I really appreciate how you handled the client call last week. Can we talk more about what made that work?” HR: “Before we jump into onboarding paperwork, I want to hear what brought you to our team. What lit you up about this role?” Coworker: “I loved your idea in the meeting! Let’s grab lunch sometime. I’d like to hear more.” > Connection is how cultures are built not just how damage is repaired. A – Acknowledge People’s Humanity Celebrate emotions, effort, and experiences. “That presentation clearly meant a lot to you. Your passion came through.” “You looked proud when you finished that and should be.” “You put a lot into this. How are you feeling about it?” > Validation isn’t just for pain. It amplifies joy, pride, and belonging. --- R – Reframe to Expand Possibility Help others see their strengths, progress, and growth. “That was brave to try a new approach. A recent K8s podcast with Clayton Coleman and Rob Shaw is a fascinating listen: https://lnkd.in/gJmpX_DB Why do standard K8s approaches don't work with LLM inference? Unlike a normal web app, LLM generates text one token at a time, each one depending on the one that came before. The technique to remember the context of the conversation is called KV caching. Now regular K8s tools for balancing traffic, adding servers and managing resources at huge scale work really well, because the amount of required varies depending on the prompt. The tools the community created to solve this problem: 🛠️  vLLM - popular inference server that is very good at KV cache management. 🛠️  Inference Gateway - smarter load balancer that enables efficient traffic distribution. 🧩 The llm-d project brings these together in a ready-to-use stack, tested and benchmarked well-lit paths, smarter routing, intelligent work splitting to optimize time-to-first token, methods to run "Mixture-of-Experts" models across multiple servers

In a personal project of mine this turned out to become a rhythm: build quickly for a while, then take some time to reconcile understanding / pay of cognitive debt. Easy to do in a low-stakes context, more interesting (ie, challenging) when considering other points on the stakes spectrum.

The AI in Healthcare Summit brought together leaders, investors, operators, and visionaries in an action-packed day. To cap it off, Vinod Khosla and I chatted live about the future of healthcare. Here's what stood out to me: 1) Healthcare for $1 a month: Imagine a world where a dollar a month (or less!) covers AI-first primary care. That's what Vinod proposed to the Prime Minister of India last week. 2) One drug, one person: developing an antibody for one cancer patient has become possible. With AI driving costs down, Vinod thinks this will become routine by 2030. 3) The always-on therapist: An AI therapist that's available 24/7, tuned to clinical-level findings, fully understanding of your specific context and needs, at a price that's accessible to all. Zooming out, our conversation made it clear that we're on the cusp of fundamental changes in how medical practitioners and systems treat patients. Today we’re introducing OfficeQA, a new benchmark grounded in ~89,000 pages of U.S. Treasury Bulletins that reflects the complex, document-heavy tasks enterprises actually face. Unlike existing benchmarks, OfficeQA measures economically valuable, real-world reasoning: parsing dense tables, navigating scanned PDFs, and retrieving facts across decades of documents. Even strong agents reach only ~45% accuracy, showing how far the field has to go. The benchmark is now open to the community, and the Databricks Grounded Reasoning Cup in Spring 2026 will challenge teams to push these capabilities forward.
