As digital health and coaching platforms continue to grow, I think there’s an important conversation worth having. Technology and coaching support can be incredibly helpful tools for many people living with diabetes. But diabetes is also deeply personal and often far more complex than scripted check-ins, reminders, or generalized advice. Sometimes what people really need is not more information. They need someone who can recognize when they are overwhelmed, discouraged, fearful, exhausted, confused by conflicting advice, or struggling to make recommendations fit into everyday life. That kind of support often requires listening, experience, critical thinking, and the ability to adapt to the individual—not just the condition. Education matters. Compassion matters. Human connection matters. For many people living with diabetes, individualized support and services still make meaningful differences.

We’ve been offering neurofeedback at Engage for the past few months, and it’s now part of what’s included in our all-inclusive tuition. Alzheimer's Disease International The diagnostics revolution in Alzheimer’s disease is a structural inflection point for global health systems. But the caveat is: If diagnostics outpace infrastructure, who benefits and who is left behind? A blood-based biomarker is only revolutionary if it is: 🔺 Affordable in low- and middle-income countries 🔺 Integrated into primary care pathways 🔺 Linked to reimbursement models 🔺 Paired with caregiver navigation systems 🔺 Embedded within national dementia strategies Otherwise, we risk building a precision-diagnosis era layered on top of 20th-century delivery systems. The true revolution is not simply earlier detection. It is the redesign of systems around what early detection makes possible. 🔺 Early diagnosis without prevention pathways increases anxiety. 🔺 Early diagnosis without treatment access increases inequity. 🔺 Early diagnosis without financing reform increases fragility. The real transformation happens when diagnostics become: ➡️ Economic tools (informing workforce planning and actuarial forecasting) ➡️ Policy levers (triggering early-care pathways and prevention funding) ➡️ Care-navigation engines (activating support before crisis) ➡️ Investment signals (de-risking therapeutic pipelines) The next frontier is not “Can we detect Alzheimer’s earlier?” It’s: Can we translate detection into systemic advantage? This requires: Integration with WHO NCD frameworks ↳ Alignment with national insurance systems ↳ Digital infrastructure for longitudinal data ↳ Caregiver inclusion from day one ↳ Ethical governance for biomarker deployment The diagnostics revolution is not about prediction. It is about preparedness. If February 26th marks the conversation, let it not end at the laboratory. Let it move into ministries, budgets, workforce models, and community clinics. Great conversation with Claire Small this week about how we can better support the MSK community beyond clinical skills. There’s a real demand for personal growth support through initiatives like Unbound Project, MSK Partners Network and events focused on business planning, culture, communication, and staff and client retention. One key takeaway: candour matters. Honest conversations drive progress. A few weeks into running Luma fertility clinic and leading the care team, we’ve been doing a lot of reflecting. We recently had our all-hands meet to look back at the last few weeks and talked about what’s working, what we’re still figuring out, and what we’ve learned so far. Here are a few observations and reflections from the field: 𝟭. 𝗧𝗿𝘂𝘀𝘁 𝗶𝘀 𝘁𝗵𝗲 𝗵𝗮𝗿𝗱𝗲𝘀𝘁 𝗮𝗻𝗱 𝗺𝗼𝘀𝘁 𝗶𝗺𝗽𝗼𝗿𝘁𝗮𝗻𝘁 𝗰𝗼𝗺𝗺𝗼𝗱𝗶𝘁𝘆 𝗶𝗻 𝗳𝗲𝗿𝘁𝗶𝗹𝗶𝘁𝘆 𝗰𝗮𝗿𝗲. As a new clinic, we don’t expect people to trust us instantly. But when they do, it’s a privilege that we do not take lightly. We’ve had patients tell us they chose us because of the strong clinical team, the care protocols we follow, and the faith in transparent and ethical lab processes. That trust is hard-earned, and we should never take it for granted. 𝟮. 𝗔 𝗹𝗶𝘁𝘁𝗹𝗲 𝗴𝗼𝗲𝘀 𝗮 𝗹𝗼𝗻𝗴 𝘄𝗮𝘆. A care team member shared how just remembering a patient’s travel schedule and adjusting her cycle plan around it made her feel seen and understood. It’s a small gesture but it made a big difference. 𝟯. 𝗬𝗼𝘂 𝗰𝗮𝗻 𝗻𝗲𝘃𝗲𝗿 𝗱𝗼𝗰𝘂𝗺𝗲𝗻𝘁 𝗲𝗻𝗼𝘂𝗴𝗵. We’re constantly making notes. What needs to be done, what could’ve gone better, what the patients’ met and unmet needs are. From care checklists and blood markers to care preferences and next steps, everything gets documented so we can build better systems and smoother journeys. 𝟰. 𝗧𝗲𝗰𝗵𝗻𝗼𝗹𝗼𝗴𝘆 𝗶𝘀𝗻’𝘁 𝗷𝘂𝘀𝘁 𝗮𝗻 𝗲𝗻𝗮𝗯𝗹𝗲𝗿 𝗮𝗻𝘆𝗺𝗼𝗿𝗲. Our EMR, our app, our backend tools don’t just support the care, they are the care in many ways. We’re learning just how deeply integrated tech is in good healthcare. While we strive to make fertility care deeply human, we also try to make it efficient by leveraging technology to automate processes. 𝟱. 𝗡𝗲𝘄 𝗮𝗴𝗲 𝗵𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲 𝗻𝗲𝗲𝗱𝘀 𝗻𝗲𝘄𝗲𝗿 𝘄𝗮𝘆𝘀 𝘁𝗼 𝗿𝗲𝗮𝗰𝗵 𝗽𝗮𝘁𝗶𝗲𝗻𝘁𝘀. The “tried and tested” patient acquisition channels may not work anymore. Just like protocols and treatments evolve, so do how patients discover and choose care. We’re trying to find ways to meet them where they are- online, offline, and everywhere in between. We’re still learning. We’re still building. But if the last few weeks are anything to go by, we’re on to something truly meaningful. If you’re building in healthcare, always happy to chat and learn more things from the field.

New episode of Living Medicine just dropped! As more clinicians and healers turn to ketamine, MDMA, and other medicines to heal trauma, the line between therapeutic experiences and therapy often blurs. How can practitioners ensure that these experiences heal rather than harm? According to trauma clinician Karen Sprinkel, trauma healing requires both depth and discernment. She cites experiential training, client choice, and flexibility as the pillars of safe psychedelic work. Karen also encourages clinicians to recognize trauma across cognitive, somatic, and spiritual dimensions and to prioritize integration and humility in their practice. Effective healing is intentional, informed, and compassionate. In this episode of Living Medicine, Dr. Sandy Newes sits down with individual and family trauma therapist Karen Sprinkel to talk about trauma-informed psychedelic care. Karen explains what defines a trauma specialist, how to safely prepare clients for psychedelic sessions, and the importance of flexibility and client agency in healing trauma.

This transition can span 30 to 40 percent of a woman's lifespan, meaning how it's navigated (and the years that follow) can really impact long-term healthspan. We’re working to help change the narrative through personalized care, advanced lab testing, and evidence-based nutrition and lifestyle strategies — so more women feel supported, understood, and empowered to thrive through this transformation and beyond.

A few weeks ago, we brought together doulas, birth professionals and prenatal care enthusiasts for our first FeelingMoms gathering. What made the evening meaningful wasn’t just the conversations, it was the shared realization that something important is missing in how we currently support pregnancy. Modern prenatal care focuses heavily on the physical development of the baby, but again and again, the professionals in the room confirmed what many mothers already feel: emotional support during pregnancy remains largely underserved. At the same time, there is a growing awareness that a mother’s emotional state during pregnancy matters, not only for her well-being, but also for the developing baby. The energy in the room made one thing clear: there is deep excitement around making emotional support more accessible to mothers, in ways that fit naturally into their daily lives. That vision is what inspired Feeling Moms. We are building tools that help mothers regulate stress, connect with their babies, and experience pregnancy with greater presence and support — drawing from prenatal psychology, nervous system science, and embodied practices. Seeing so many professionals show up and engage in this conversation was a powerful reminder that this work is bigger than one platform. It’s a growing movement toward supporting mothers more fully during one of the most important windows of human development.

As we approach the completion of the technical and administrative tasks for this important NIH-funded feasibility study, and with IRB approval granted, we are excited about the upcoming patient recruitment. We look forward to seeing this innovative technology implemented in skilled nursing facilities (SNFs) as we take a first step into the RI Life Science Hub.   Playing the "right music at the right time" is no longer just a catchy marketing slogan; it is now a genuine clinical intervention that has enormous potential to benefit both people suffering from dementia and their busy caregivers.   Thank you to the Brown team: Ann R., Rosa Baier, Ellen McCreedy and Rubato's team, led by Richard Koffler and Noam Guy, with clinical support from Zoe Schuver and the rest of us who contributed to this project. At Allara, we don't just provide care: we're advancing the science of women's health. Last year, we partnered with UCSF's Dr. Heather Huddleston to conduct the first-ever comprehensive study examining how PCOS impacts women's professional lives. This research filled a critical gap in healthcare literature that has existed for decades. The study's scope was unprecedented: Over 1,100 women across North America participated, providing the largest dataset ever collected on the intersection of hormonal health and workplace productivity. What we discovered challenges how the healthcare industry thinks about women's health outcomes: >> 50% of respondents reported missing work due to PCOS >> 72% said PCOS affects the quality of their work >> 51.5% feel held back in their careers because of their condition This research represents a fundamental shift. We're generating the evidence base that should have existed decades ago. Every patient interaction at Allara contributes to our understanding of how women's bodies actually respond to different interventions. This data informs better treatment protocols and outcomes.
