Everyone deserves the chance to heal with dignity – especially those battling opioid addiction. UC San Diego Health, together with the The City of San Diego, is expanding the Transition Support Team Pilot Program, a powerful initiative that connects individuals in crisis with compassionate, coordinated care. Launched in 2024, this program has already helped hundreds of patients begin their recovery journey by meeting them at a critical moment in the emergency department and surrounding them with the support they need to heal. Bolstered by the pilot program’s initial impact, additional funding from the City’s Opioid Settlement allocation will allow us to expand this model to reach more people, introduce new roles like psychiatric nurse practitioners and psych pharmacists, and extend services to our most impacted communities. This is what it looks like when we unite across sectors to solve complex challenges. Together, we are reducing readmissions, restoring hope, and building a stronger, more resilient continuum of care.
