I think having a backup is also important. I agree that we should do all mentioned six steps. Sometimes after doing all these steps also problem prevails. Keeping a backup somehow makes this process better and helps to get work done. So, keeping a backup is additional step to above and example of it could be extra time or extra team member just in case.

An important step is also to assess your own role in the situation. Was there something we did or didn't do because of which the person was not able to keep up the commitment. An important thing to also reflect is whether the person was 'unable' or 'unwilling' to cooperate which led to the breach of trust. Like understanding whether it was a competence lapse, character issue or plan apathy.

Move forward in your actions, but always close out the subject as well. At a certain point it is important to revisit the topic. For me, in a one on one, I have asked questions like "I know we had a challenge in our level of trust. We spoke to it, agreed to actions to prohibit it from happening again, but I want to ensure you feel like we are past it in the same manner that I am feeling". If you don't agree to close it out, there is risk of "remember when you" being a topic going forward. Closing it out allows for that to be immediately addressed.
