The first step would be isolating the affected area and restrict movement and operations in that area. The second step would be addressing the first-aid/medical requirements to people if required. Third step would be identifying the hazards, take immediate corrective action to remove those hazards and re-start the operation in a controlled manner. Then launch a thorough investigation by HSEQ lead or EHS lead and also review the near-miss register to understand the probability of occurrence of this unwanted incident. Once the analysis and investigation is over, take permanent corrective action and also re-train people on the safety measures and failure modes. Keep a close watch on near-miss incidents to ensure effectiveness of actions.

How I've always approached this, simple but effective. 1. Secure the area (get control of the scene). 2. Determine the root cause. (Detailed investigation) 3. formulate the corrective action plan. 4. Drill wide, quickly determine if the situation can replicate itself anywhere in facility.
