Step 4: Your participation in the program is now complete! Please look for emails from us that will contain two things:

• A monthly summary of your accrued contributions
• Instructions on how to redeem your digital credits for use in the Charity On Top Foundation’s charity marketplace
  

Customers must opt in to the program within 30 days of purchasing and activating a new line of service on an eligible Boost Mobile plan from boostmobile.com. 

There are zero extra costs to you or the nonprofits. As long as you pay your monthly bill, we’ll set aside 5% of each eligible wireless phone line (Excluding device charges, taxes, and fees) each month. In the following month after a regular calendar quarter, Boost will send a unique code for you to access your digital credits for use in the Charity On Top digital marketplace featuring nearly 2 million causes.

Most Boost Mobile plans from boostmobile.com are eligible. However, Infinite Access plans and the Free Year of Service promotion do not qualify. Any purchases made outside of boostmobile.com are ineligible at this time.

Your contribution is calculated at the end of each quarter, and you will receive a digital code to access your credits on Charity On Top Foundation’s charity marketplace to the primary email address on your Boost Mobile account. This e-mail should arrive approximately 30 days following the end of the quarter close and you will see a Contribution Amount that you can use (The minimum accrual amount required for a digital code is $5. If you don’t exceed this minimum, then your accrual is carried over to the next quarter.). You will have 90 days to make a selection. If you don’t use any or all of your digital credits within this time period, then Boost will credit these to the Charity On Top Foundation’s General Fund.

If a participating customer leaves or disconnects any wireless phone line at any time, then they will automatically surrender their earned Contribution Amount that has not already been distributed. Surrendered Contribution Amounts will be donated directly to Charity On Top’s General Donation Fund.

Eligibility for Boost CARES is determined by the following:

• Opt-in to the program: All customers must share their info via this form above to participate in this program

• Order Status: New wireless phone lines ordered through boostmobile.com only and activated within 30 days

• Order type: Wireless phone lines only (Excludes: Infinite Access, Free Year of Service, Tablets, Watches)

• State of Residence: The program is currently not available if you reside in one of the following states: Alabama (AL), California (CA), Hawaii (HI), Illinois (IL), Massachusetts (MA), Mississippi (MS), Missouri (MO), New York (NY), South Carolina (SC), or Virginia (VA).

• Account Status: Customer accounts whose bills are not current or settled, including suspended account

Many names may appear, so you can also use the tax-ID number to find your organization directly.
