You can download the forms you may need as a Way Forward client below.

Some of these forms are signed and managed electronically through our client portal. These are the:

• Letter of Authority – which allows us to speak to your creditors
• Direct Debit Request – which provides us with authority to debit funds from your nominated bank account.

Target Market Determination – this outlines the group of people that our service is designed to help and explains that we offer a direct debit facility as part of the payment plan.

Credit Guide, Combined Financial Services Guide and Disclosure Statement – the Credit Guide explains information about the credit assistance we provide, and the Combined Financial Services Guide and Disclosure Statement describes the financial products and services we offer.
