Another interesting read after Endpoints News released their latest BPSI showing a notable improvement in industry confidence heading in to H2, a stark contrast to market conditions 12-18 months ago, with optimism largely driven by: 1. Increased M&A and licensing activity 2. Renewed capital flow 3. Continued momentum around AI 4. Companies pushing assets toward meaningful clinical inflection points. Of course, not all rosy, with the report also highlighting a cautious undertone beneath the optimism. Regulatory uncertainty, political pressure, pricing concerns, and broader market volatility still appear to be keeping many organizations disciplined in how they scale. From a talent perspective, it does feel like the market is shifting from 'survival mode' back toward selective growth. Cautiously optimistic probably feels like the right way to describe the current state of biotech right now. Biopharma does not suffer from a lack of insight. It suffers from underutilised insight. Across the industry, valuable field intelligence too often disappears into disconnected systems - collected, reported, and archived without materially shaping strategy. But insight only creates value when it drives action. Following on from this post: Insight that drives action has to start somewhere. I'd argue it starts with the patient, and more specifically, with the moments in their journey where their choice or experience disproportionately shapes the outcome. Call them moments that matter. Get those right, get to the insight behind them, and the strategy almost writes itself. The catch: those moments cannot be identified by one function in isolation. Commercial will name different moments than medical. Market access and/or sales will name different ones again. Patient services will see things the rest of us miss entirely. So before the patient journey workshop, the more useful question isn't what are the moments that matter? It's who needs to be in the room before we can credibly answer that? Coherent strategy starts there.

The competitive advantage of choosing a clear starting population One thing I value in early innovation is specificity. Not in molecules. In thinking. In discussions around AML, I’ve heard some of the strongest and most focused conversations, where teams could clearly explain: - Where the unmet need is - Which patient segments matter most - Why a mechanism is relevant for that biology That clarity is a competitive advantage. Too many early teams stay broad for too long because it feels safer. But broad positioning can become vague positioning, and vague positioning slows decisions. In 2026, I expect sharper choices earlier: - Clearer patient definitions - Better biomarkers and stratification logic - More explicit “why this group first” Even if you plan to expand later, choose a starting point that makes sense scientifically and clinically. If your program had to start with one patient population only, which one would you choose and why?
