Your AI agents can read your files, call your APIs, and talk to the internet. That's the "lethal trifecta" — and it's why prompt injection is the attack vector most enterprises still aren't taking seriously. An attacker plants a malicious instruction in a document, email, or webpage. Your agent reads it and executes it with your credentials. You never see it happen. Sophos CISO Ross McKerchar just published one of the clearest, most actionable pieces I've read on this: seven concrete patterns for containing the blast radius of a compromised agent, right now, without waiting for the tooling to catch up. The one that stuck with me: assume the model will be compromised. Stop trying to prevent every injection. Start thinking about segmentation, credential isolation, and audit trails, the same mental model we applied to endpoint security a decade ago. The Canvas disruption during finals week, paired with extortion messaging, was a sharp reminder that operational chaos shows up fast when access, privilege, and ownership are unclear. The first question after any disruption is still simple. Who has access to what. That is usually where the real story starts. The failure mode is rarely a lack of AI. It is unclear identity ownership, stale privilege, and missing accountability. AI just accelerates whatever environment it lands in. If the identity layer is messy, the mess happens faster.
