This is so true and it’s something I was just discussing with a family in La Jolla. For them, every afternoon felt like a total guessing game, and that unpredictability is where the highest stress comes from in dementia care. I always tell my teams in Mission Valley, Old Town, Point Loma and across Central San Diego that these routines are not about being rigid. Instead, they are about providing an anchor. When the world feels like it is shifting, a predictable schedule tells the brain it is safe. Keeping the big rocks of the day like meals or a morning walk at the same time helps lower that baseline anxiety. A simple way to start is to pick one small thing, like a 10:00 AM cup of tea, and make it a non-negotiable. We often talk about dementia care in terms of what has been lost. But some of the most meaningful progress I’ve seen happens when we shift the conversation toward what remains. Abilities don’t disappear all at once — they change, adapt, and express themselves differently. When care teams understand how a person continues to learn, communicate, and engage, environments begin to feel calmer, communication becomes more natural, and daily routines become more meaningful. Ability-based thinking isn’t about ignoring challenges. It’s about giving professionals a structured way to see strengths that are still present — and building support around them intentionally. As more organizations explore innovative environments, community partnerships, and new models of memory care, I believe the next evolution will be pairing thoughtful design with deeper insight into preserved abilities. Because truly person-centered care doesn’t start with limitations. Families caring for people with dementia often feel overwhelmed and unprepared. Each family has their own unique needs, but all of them need guidance on what to do, what to expect, and how to prepare for the future. The Johns Hopkins University, in partnership with the Maryland Information Network (MdInfoNet) and Maryland Department of Aging, has developed a tool that can help. The Johns Hopkins Memory Care Family Checklist is an innovative resource that helps families impacted by dementia identify their needs, access reliable information, and connect to supports to help them develop a larger care plan.
