The HBR article on "AI nightmares" provides an insightful framework on proactive identifying risk, especially in the larger organizations with structured teams and governance. But it made me think about smaller firms that don't have the formal committees or dedicated AI oversight. They usually don't have time to build them. What they do have is speed: less red tape and faster decision-making. These smaller firms are operating more reactively than proactively. The thing is, AI risk doesn't scale with company size. The exposure is the same. They will come across bias, misinformation, client-facing errors, etc. So, while the structure in the article may not translate directly to the smaller firms, the core idea does: - Ask what could go wrong - Discuss perspectives - Get ahead of the risks It doesn't have to be complicated. These conversations need to happen, especially when about 78% of organizations use AI.
Most companies’ responsible AI strategies were built for a different era. In the age of generative AI and autonomous agents, policies and principles alone often move too slowly and leave too much open to interpretation. A more practical approach starts by identifying your organization’s AI ethical nightmares—the scenarios that could seriously damage trust, operations, or reputation—and working backward from there. The Ethical Nightmare Challenge reframes governance around three questions: What could go wrong? What safeguards need to exist? And how do you prepare employees to act effectively when risks emerge? By focusing on concrete worst-case scenarios, leaders can create AI governance systems that are faster to implement, easier to communicate, and more adaptable as the technology evolves. When AI works, it does better than humans. When ai fails, it introduces harm in many ways. In high stakes applications, people die, lawsuits ensue and trust is eroded. Ai standards are exclusively internal. When speaking to ai companies, they follow compliance standards to the letter. But compliance standards such a HIPAA don't regulate AI, they regulate privacy and patient rights. They do not cover ai/patient interactions and what happens when ai gets it wrong. Yes, I believe that this trajectory will continue to improve and that ai will become more competent than humans in life or death situations such as these. But the regulatory conversation around ai completely misses the mark on how ai works on a fundamental level.

What are your AI nightmares? What would catastrophic AI failure look like in your business? What are the scenarios procurement, regulators, or customers would never forgive?
