- [Instructor] In order to improve for good, you need to solve problems at the systems level. Fix the inputs and the outputs will fix themselves. This is a quote by James Clear in his book Atomic Habits. Similarly, that is what the 3 x 5 Why can help you do. As the name implies, the model is composed of three distinct legs that follow a cause and effect chain. For each leg, though, the types of questions that help lead to a deeper root causes are different. For example, the first leg is the Specific leg and it's meant to help analyze how the problem was not prevented. The questions should determine the direct cause, why the situation occurred in the first place. The causes in this leg typically point to something missed related to your quality assurance processes.
