- [Narrator] If you've ever experienced a bit of chaos when an accident happens, it can often be due to a lack of clarity in roles and responsibilities. While I've seen best practices insofar as first responders taking care of persons impacted, the rest of the process meant to prevent a similar accident from happening again can have gaps, many of which are due to role confusion and an unclear understanding of the flow of activities. A swim lane diagram can help ensure everyone involved knows their role and the process sequence. For such cases, let's build a swim lane as an example. Let's add the roles, first responder, supervisor, investigators, cross-functional root cause analysis team, operations, and change management team. Notice that it's anyone's role to sound the alarm in case of an incident. We represent that activity with a circle. This alarm is actually the first handoff to first responders.
