Doctors and nurses giving people the wrong medication or the wrong dosage of medication harms 200,000 Americans every year. Now, for decades, hospitals have just thought, this is the cost of doing business. There's nothing they can do about this problem. Until a brave group of nurses tried to get to the bottom of why so many people are harmed by this perfectly preventable problem.
