I was so glad to participate in this insightful study with Kyra Gardner and Andy Wang as part of their program at Columbia led by Halle Tecco, MPH, MBA. I do believe AI will massively accelerate healthcare innovation, but right now it is stuck in a hype cycle and much of the early AI applications in healthcare just seem to be making the status quo more efficient rather than really transforming the system. We will get there....but there will be bumps and bruises along the way.

Two weeks ago on Swimming with Allocators, we sat down with David Clark, CIO at VenCap, to talk about what decades of venture data can teach allocators. One takeaway that stood out: discounts don’t matter as much as people think in venture secondaries. Because venture is such a power-law asset class, outcomes are driven by exposure to a few massive winners. Whether a stake is bought at a small discount, or even a premium, often matters far less than the quality of the underlying company and its upside. Big step forward—congrats to the team. This is exactly the kind of signal that the market (and policymakers) are ready for a more connected, patient-facing model. Bringing dental into the broader health ecosystem has been a blind spot for a long time, despite how critical oral health is to overall outcomes. The combination of conversational AI + real-world access + cost transparency is where things start to get very interesting—not just for engagement, but for actually changing behavior and improving prevention. Distribution is becoming the moat in healthcare AI Anthropic’s recent Claude for Healthcare update was a good example: the main features were not focused on model performance, it was connectivity, making Claude usable inside the healthcare enterprise stack. The main drops were: -> Coverage rules (CMS Coverage Database connector): check whether a treatment is reimbursable and under what conditions -> Coding (ICD-10 connector): translate clinical language into billing/diagnosis codes -> Provider verification (NPI Registry connector): validate provider identity for credentialing and claims -> Prior Authorization Agent: assemble documentation and rationale for approvals/appeals -> Interoperability tooling: make it easier to plug into health IT systems This shift creates real opportunity for founders. Healthcare is fragmented by country, system, and workflow, so teams that can embed applications deeply in specific workflows can build meaningful markets, even from narrow starting points. Anthropic launched Claude Managed Agents. And woven into the announcement is a structural shift that every enterprise technology leader should be paying attention to. The pitch is straightforward. You define tasks, tools, and guardrails. Anthropic runs your agents on their infrastructure. Sandboxing, state management, credential scoping, execution tracing. All handled. But here's what matters more than the speed. This is a reference architecture that treats the model as the runtime. Not as a feature bolted onto existing software. Not as a copilot living inside someone else's UI. The model IS the execution layer. Anthropic owns the harness, the orchestration loop, the infrastructure. You bring the workflow logic. And because the harness abstracts the model boundary, every upgrade flows straight into your agents without re-architecture. For legacy software companies, this is the threat they have been slow to name. Every generation of enterprise software accumulated technical debt in the form of brittle integrations, rigid schemas, and custom orchestration code. That debt now becomes a moat in reverse. It is the thing preventing you from absorbing the next model upgrade cleanly. What Anthropic is offering here is model-as-a-service. Not a model you call. An entire execution layer you subscribe to. And every time the model gets better, your agents get better. Without a re-architecture. Companies building using this architecture do not carry that weight. They can swap in Mythos tomorrow and inherit the capability jump without re-architecting anything. The managed agent harness abstracts the model boundary. That is the whole point.
