Standing up a high value advisory board is part art, part architecture. My process always starts with a charter — a one-page document that defines purpose, member expectations, cadence, and outcomes. Before recruiting anyone, clarify: • What’s the problem this group exists to solve? • What decisions will you make based on their input? • What commitment do you expect — quarterly meetings, informal availability, pilot feedback? The clearer the purpose, the stronger the engagement. A great board doesn’t just happen. Lots of insightful thoughts from Robbie Allen throughout our conversation, but a few in particular have lingered with me the most when I think about the evolving role of MSOs in our healthcare delivery ecosystem… 🔹The often overlooked potential health equity value of MSOs US Heart & Vascular has built a meaningful presence in Louisiana and Alabama, two of the top five states in terms of the highest heart disease mortality rates (CDC). 🔹It’s great to see a national MSO investing in state-of-the-art excellence, patient service, patient care, and cost of care, not only reaching underserved markets but actually focused on addressing gaps in care in these markets as a core part of their business plan. 🔹When you step back and look at the disparities in healthcare access and outcomes across states, it becomes clear: MSOs may have a bigger role to play in driving positive health equity outcomes than we often acknowledge. Kudos to US Heart and Vascular for setting an important example. 🔹The international opportunity is coming We’re still early in this conversation, but momentum is beginning to build. 🔹This episode adds to a growing list of MSO CEOs who are now actively contemplating expansion beyond the U.S. as a legitimate avenue for growth. 🔹What makes for optimal market dynamics? There’s no single formula, but the cardiovascular space stands out for a few reasons: – A compelling value-based care opportunity serves as a catalyst for investment. – The execution complexity of VBC creates higher barriers to entry – The expanding shift to outpatient care and the growing migration of providers from hospitals to independent platforms create an increasing pool of opportunities. Right now, the availability of white space is enabling a healthy mix of competition, peer collaboration, and investment in long-term value creation. It makes me wonder…regarding the role that available untapped opportunities can play in creating positive market dynamics: Could international expansion eventually become the next white space opportunity across a broad range of specialties? TAKEAWAY FROM THE STRATEGY CONFERENCE One insight that emerged from the SESHSMD (Southeast Society for Healthcare Strategy & Market Development) annual conference is that many health systems are following a trajectory similar to Ray Kroc’s McDonald’s Restaurants. You see, Ray Kroc acquired many of the original McDonald’s locations to secure the best sites against competitors. At a certain point, his business wasn’t just about food—it was also a real estate business. Health systems today are facing a similar dynamic. As they continue to merge, acquire, and grow, they too are effectively real estate businesses. With the timing and location of new sites critical to success, health systems now need a more proactive, strategic approach to capital, site selection, and market growth.

For too long, our system has focused on the volume of services provided, rather than the quality of care delivered. This approach has led to higher costs and inconsistent outcomes. I recently participated in a CEO Panel at the AHIP Medicare, Medicaid, Duals & Commercial Markets Forum, where we discussed the future of health care delivery innovations. One of the key takeaways from our conversation was the importance of shifting toward rewarding quality over quantity to improve health outcomes, reduce unnecessary costs, and create a more sustainable health care system. By incentivizing providers based on quality rather than quantity, we can create a more sustainable and effective health care system. This approach, which we've implemented through strong partnerships in our provider networks, has led to improved health outcomes and reduced costs for our members. For example, the value-based Sanitas care model at our Florida Blue | Sanitas Medical Centers creates more seamless and coordinated care for our members that prioritizes prevention and early intervention. As a result, we've seen measurable reductions in avoidable hospitalizations and improvements in chronic disease management. I'm proud of the work we're doing at GuideWell to create a more sustainable and effective health care system. Yesterday, after a brief hiccup when #MA #StarRatings appeared on MPF for a few minutes, Humana issued an 8k about 2026 #Stars. Headline: the nation’s largest #MAPD plan (H5216) did not regain 4+ stars. This is not: ❌ A signal of worse quality, customer service, or #memberexperience. ❌ A failure of Stars tactics or cutpoint models. It is: ⭐ The inevitable result of decade-old baggage. In the mid-2010s, nationals consolidated contracts to optimize #QBP. Though #CMS closed those loopholes long ago, the resulting baggage of huge contracts lingered. If you weren’t in #MedicareAdvantage back then, here’s a summary: https://lnkd.in/etvDiBB8 ⭐ A reminder that size matters. The larger a contract, the slower the change. Some contracts are too big for quick projects. Some notable details include: 1️⃣ TOP QUARTILE TARGET With Reward Factor gone, #HEI out of reach and continued measure changes, H5216’s lack of control over providers, 50-state presence, and competition from others makes even top 25% an uphill climb. 2️⃣ DIVERSIFICATION TO DE-RISK Getting a new H # is hard. De-risking is occurring via market/pbp exits and reduced commissions in this PPO, alongside offers of other products as alternatives. Headwinds will continue after VBC/risk-bearing providers see their first lost premium revenue due to H5216's lost PY26 QBP. 3️⃣ MY24 3.61 AVERAGE Last year’s drop was never about 1 FL/TTY call. It was structural and inevitable. The next data point to watch: MY25 raw rating. If it falls, contract size is the barrier regardless of strong tactics, great staff or high-quality providers. HUM could react with radical transparency (publicly sharing real-time ratings data and cutpoint models) to rally combined efforts of all providers and help investors price QBP implications real-time. 4️⃣ REALIGNING DISTRIBUTION > NETWORK NARROWING Stars impact of enrolling unengaged or difficult-to-care-for members, not just provider quality, matters. Adjusting consumer/product fit is far wiser than removing providers from networks. Some tips as we await ratings release: ✅ HUM's detailed transparency may result in others being asked to do the same. CEOs need a direct line to the Star leader and detailed understanding of contract/provider performance, cutpoint assumptions and #PathToGoal. ✅ If a Stars team is 3+ layers down from the CEO, change it. Every C-suite should receive unfiltered, transparent information. ✅ Don't let H5216's unsuccessful MY24 Q4 push discourage you. H5216 is a cruise ship, not a speed boat. Most plans can pick up .5-1 star in the Q4/Q1 push.
