Three reasons your AI pilot died. Only one of them is the AI. I've now reviewed dozens of failed AI rollouts inside SMEs. Different industries, different tools, different team sizes. The pattern is almost always the same. The AI worked. The rollout didn't. Here are the three killers, in the order they show up: 1. The Pilot That Goes Nowhere. Someone signs up for ChatGPT Plus. Uses it for a week. Gets excited. Shows the team. Gets busy. Stops. No measurement. No rollout. No second user. Three months later nobody can remember what the original use case was. Cause of death: enthusiasm, with no operating model behind it. 2. The Shiny Tool Purchase. A vendor pitches the MD a platform document automation, scheduling AI, CRM intelligence, that sounds impressive in the demo. The cheque is signed before anyone has asked the harder question: is this actually our biggest problem? The tool gets bought. Half-implemented. Quietly abandoned by month four. The licence keeps renewing for another two years because nobody's brave enough to admit it's not being used. Cause of death: solving the wrong problem, expensively. 3. The Paralysis Loop. Leadership knows AI matters. Every meeting produces a different suggestion. Marketing wants to use it for content. Operations wants it for SOPs. Finance wants it for forecasting. Sales wants it for prospecting. Nothing gets prioritised. Nothing gets resourced. Months pass. Competitors move ahead. Cause of death: democracy without a decision-maker. Here's the uncomfortable truth most AI vendors won't say: The AI is rarely the problem. Hallucinations get fixed. Models get better. Pricing comes down. What doesn't fix itself is the adoption infrastructure, the rollout plan, the measurement, the prompt library, the training cadence, the person whose job it is to make sure the tool actually gets used. Most SMEs don't need a better AI tool. They need someone who's been through enough failed rollouts to know what kills them. Half your sales team's day is gone before lunch. Not on selling. On finding people to sell to. I sat across from a Sales Director in the Midlands last month. Good business. Growing. Ambitious. But his team wasn't converting fast enough and he couldn't work out why. I asked him to walk me through a rep's typical morning. Arrive. Check email. Open LinkedIn. I was 6 weeks into an operational improvement programme with a construction chemicals manufacturer when the Quality Manager flagged something I wasn't expecting. "Graeme, I need to talk to you about our Safety Data Sheets." She looked exhausted. Not the usual production pressures. Something else. "We've got 130 products that all need new SDS documents. Every single one has to be transferred from the supplier's format into our branded template. 16 sections. 63 subsections. Every data point copied manually." I asked how long each one takes. "About 45 minutes. And that's on a good day." I grabbed my calculator. 130 products × 45 minutes = 97 hours. That's over 12 full working days of copy-paste. For one person. Doing nothing else. She'd been through this cycle before. Every time a supplier updated their SDS. Every time a new product was added to the range. The same 12 days of grinding, repetitive data transfer. I was on-site with a client last month doing an operations coaching session. Between meetings, their Operations Manager pulled me aside and said something I hear all the time: "I spend half my day just doing emails. I can't get to the actual work." I asked how many emails he gets a day. "About 120. Sometimes more." I asked if he'd ever used Microsoft Copilot in Outlook. He looked at me like I'd spoken a different language. "I've heard of it.
