Kidney care is about more than medical treatments; it's about ensuring every person has the resources and support to live their life to the fullest. Our care coordinators assist with scheduling appointments, finding durable medical equipment, exploring home health options, and researching community resources when insurance doesn't cover a need. When one of our patients needed a hospital bed for use at home, her Interwell care coordinator helped secure a bed through a local community resource at no cost to the patient. A recent survey from AHIP and the Centers for Medicare & Medicaid Services confirms a resounding vote of confidence in alternative payment models (APMs), including value-based care, as health plans continue to move away from fee-for-service models. ⚕️Nearly half of all healthcare payments were tied to APMs in 2024. 📈 70% of respondents expect APM activity to increase over the next 24 months. 🪴 55% of respondents believe shared risk/procedure-based episode payments will be the fastest growing APM. New research from UVA Health reveals damaged kidneys release particles that harm the heart, explaining the strong link between CKD and heart failure. This direct connection underscores an important point: CKD can’t be managed in isolation. Yet traditional fee-for-service models don’t support the additional resources and services needed to deliver holistic kidney care. Providers and payers are increasingly adopting value-based care models that emphasize interdisciplinary, coordinated care to improve patients’ overall health and long-term outcomes. New research from Renal Research Institute (RRI) shows AI-driven Imminent Hospitalization Predictive Models (IHPM) are able to predict near-term risk of hospitalization for patients on dialysis. Even more impressive, the research demonstrated an 8% reduction in the odds of hospitalization within seven days for patients who received AI-driven interventions. At Interwell, we are using IHPM alerts to identify at-risk ESKD patients and trigger timely interventions to help keep patients out of the hospital.
