States are getting better at catching Medicaid fraud before the money goes out the door, not after. DC is layering EVV GPS data on top of claims data to flag weird billing patterns. Arizona's using AI to flag high-risk claims before they're paid. MFCUs recovered $4.64 for every dollar spent last year, nearly $2B total. There's also a stat in there about Medicaid renewals—47% went through last April without anyone touching paperwork, approved automatically because the state's system could already verify eligibility from existing data. What does "HCBS" actually mean? The SCAN Foundation and Human Services Research Institute reviewed 229 definitions pulled from over 7,500 sources to build a shared, consensus definition of home- and community-based services. It's not tied to Medicaid or any single program, so it holds up across populations, ages, and funding sources. As states rethink Medicaid HCBS spending, common language makes it easier to measure outcomes, hold programs accountable, and show what's actually working. The direct care workforce is now the largest workforce in the country. There were nearly 5.8 million workers in 2025, up from 3.6 million in 2015. PHI projects an additional 886,000 new direct care jobs by 2035, with 9.6 million total job openings when you factor in turnover, which is more than any other single occupation in the US. Homecare is driving most of that growth: there are 773,000 new jobs projected through 2035, a 21% increase. But wages haven't kept pace. Homecare workers earned a median $16.98/hour in 2025, still $3.57 below comparable occupations. Additionally, federal Medicaid cuts could impact the pipeline just as demand accelerates.
