The CDPHP Foundation has announced its 2026 grant recipients — three incredible organizations working to expand access to health care and essential services across New York! 🌟 Funding will support transportation to care through Astor Services, free medical transport for older adults via Captain Community Human Services’ Wellness Express, and a new food pantry for veterans through the Veterans & Community Housing Coalition.
