She didn’t remember the day, but she remembered her kitchen. The smell of her backyard. The creak of her favorite chair. After six months in a nursing home, her dementia had progressed. Through the NHTD program, we helped them find a ground-floor apartment near her daughter, arranged home modifications, dementia-trained aides, transportation, and 24/7 support.
