Retrospectives, retrospectives, retrospectives! It's OK to get it wrong, but you simply MUST learn from it. The only way you do that is by looking back at the good, the bad, and the ugly. Don't be afraid to point out where YOU got it wrong and encourage others to do the same. Nobody is perfect but through regular reviews, we can iteratively get better. Here again, qualitative and quantitative data is critical. Say "I think..." or "I believe..." way less and instead, say, "The data shows...". If you don't have the data, get it if at all possible.
