Breaking News: Risk Assessment Failures Lead to Fatal Tram Accident in Edinburgh “When risk assessments fail, lives are lost prevention is always better than reaction.” A tram company in Edinburgh has been fined £240,000 after a series of risk assessment failures led to the tragic death of Carlos Correa Palacio, a 53-year-old bus driver, on 11 September 2018. 📊 Key Insights Fatal incident: Carlos was fatally struck while crossing the tram line on his way home from work. Delayed response: The tram driver saw Carlos 73 metres away but only began slowing and warning signals after traveling 20 metres, applying emergency brakes just 18 metres before the crossing. No proper risk assessment: Edinburgh Trams Limited failed to carry out a suitable and sufficient risk assessment for the crossing layout and pedestrian safety. Ignored warnings: A 2016 near-miss incident at the same crossing went unaddressed, leaving hazards uncontrolled. Equipment oversight: The company never assessed audible warning loudness or braking distances, preventing the implementation of safer control measures. ⚖️ Legal Outcome Edinburgh Trams Limited admitted breaching health and safety legislation in Edinburgh Sheriff Court. Investigations confirmed the tram driver followed procedures correctly and that the tram systems were functional pointing to management and system-level failings as the primary cause. 🧠 Lessons for IOSH Members Never treat risk assessments as one-time tasks, they must be regularly reviewed and updated. Respond to near-misses proactively to prevent repeat incidents. Ensure effective communication of risks and warnings to pedestrians, employees, and contractors. Offshore health and safety consultation extended People and organisations now have more time to share their views on proposals to update Manx offshore health and safety legislation. The Department of Environment, Food and Agriculture (DEFA) has extended the majority of its current health and safety consultations to 31 March 2026, to support full public and stakeholder engagement. The proposals aim to ensure robust protections for future offshore energy developments and, where appropriate, to align existing legislation more closely with UK Health and Safety Executive (HSE) standards. HSE Fatal Injury Statistics: A Stark Reminder for Safety First 🦺 The latest figures from the Health and Safety Executive (HSE) reveal a shocking reality: 124 workers tragically lost their lives in work-related incidents across Great Britain between April 2024 and March 2025. These are not just numbers, they represent lives lost and families changed forever! The most common causes of fatal injuries: • Falls from height - 35 deaths. • Struck by a moving object - 18 deaths. • Trapped by something collapsing or overturning - 17 deaths. ⚡️New HSE Principles for First Aid & Medical Care in the Wind Sector 🌍 On 22 August 2025, the HSE issued updated guidance for the wind industry, setting out new principles for how we manage first aid and medical care in high-risk, remote environments. Key updates include: ✅ Training – GWO Enhanced First Aid (EFA) or equivalent, with site-specific equipment training. ✅ Practical skills – On-the-job training and refreshers (HSE recommends annually). ✅ Resilience – Enough trained first aiders on site to respond even if some are incapacitated. ✅ Competence – Employers should use HSE GEIS3 guidance when selecting training providers. In my role with MRS Training & Rescue, I see every day how vital it is that training goes beyond certificates — it’s about building real competence so teams are ready when it matters most. The Health and Safety Executive (HSE) has issued important guidance for wind energy projects on first aid standards. A person is considered fully trained if they have completed the GWO Enhanced First Aid (EFA) course—or an equivalent qualification. Additional training is required if the first aider is unfamiliar with site-specific equipment (e.g., oxygen systems, airway management tools, stretchers). Annual refresher training is recommended to prevent skill fade. First Aid Needs Assessments (FNA) should address foreseeable risks, illness, as well as injury, and provision for delayed emergency response.
