Humble: nav2_navfn_planner poses created had uninitialized header (#5087)
* added disengage_threshold
* added global costmap clearing once it reaches goal
* fixed merge error in rotaton_shim_controller
* fixed pose header frame_id in nav2_navfn_planner
* reverted change to nav2_bt_navigator/behavior_trees/navigate_to_pose_w_replanning_and_recovery.xml
