Last week, our Director of Professional Practice, Clinical Programs, Matthew Wong attended the Community Health Nurses of Canada conference in Vancouver where he spoke about one of our most meaningful partnerships: Sinai Health to Home, a collaborative program that helps patients transition safely and smoothly from hospital to home. It was a great opportunity to highlight something we believe strongly at Circle of Care: home care is essential to the future of healthy, dignified aging in Canada. As our population grows older, the ability to support people at home — where they want to be — will play a major role in building a sustainable healthcare system. Matt shared how programs like Sinai Health to Home show what’s possible when hospitals and home care providers work together. These partnerships reduce pressure on acute care, improve patient experience, and create a more integrated system overall.
